Vehicle Mode: Ratchet’s vehicle mode is a pretty standard truck.  Nothing about it says ambulance except for the colors, so that’s pretty annoying.  They also failed to put an Autobot symbol on his grill even though there is a molded outline for one.

Robot Mode: While his vehicle mode isn’t anything special, Ratchet looks fantastic in robot mode.  He’s got great paint apps and an excellent head sculpt.  He really looks great.

Overall: I wish they actually made him an ambulance, but Ratchet is still pretty great.
